Event Details
Greater Springfield Senior Services will offer a free, one-hour presentation called Advance Care Planning: It’s About the Conversation.
The goal is to educate people of all ages about the importance of choosing a healthcare proxy, writing advanced care directives, and having these conversations with loved ones.
Light refreshments will be served, and all participants will receive a free copy of the Honoring Choices toolkit, which includes relevant Massachusetts forms.
Space is limited to 12 participants, and registration is required. Please see library staff to sign up; however, if you have any questions related to the course, please contact Lenora Johnson at 413-781-8800 ext. 1385.
